For your convenience, the Healthy Life Questionnaire will be available here from March 27 until April 8 (for instructions click here). By completing the questionnaire online, you will be able to see your lab results online after your HRA has been completed. It does not include your complete risk factors (ie: Biometrics). This service is not available for paper applicants. A 15-minute follow-up visit with the Nurse Practitioner (Vickie Warden) is required for every employee.
